• Open a New Health Savings Account

    Check that you qualify for a Health Savings Account (HSA) and apply today.
  • A Health Savings Account (HSA) can save you a lot of money on medical expenses. But before you apply for one, please check to ensure that you qualify. In order to open an HSA, you will need to:

    • Be participating in a High-Deductible Health Plan (HDHP) on the first day of the month in which you open the HSA
    • Have no other health coverage
    • Not be enrolled in Medicare 
    • Not claimed as a dependent on someone else’s tax return.

    Step-by-Step Instructions for Opening Your HSA.

     1. Read Important Disclosures:

    Please print or save all disclosure PDFs listed below.  Need help opening these files? These documents do not have to be signed or returned to us.

    2. Complete the HSA Adoption Agreement:

    Please print, complete and sign the HSA Adoption Agreement. Please be aware that only one individual may be an owner of an HSA account. 

    If you would like to allow someone else to withdraw funds, please complete the "HSA Authorized Signer Designation" on page 1 of the HSA Adoption agreement and have the person sign that form.


    3. Sign HSA Signature Cards:

    Print, complete and sign two HSA Account Signature Cards.


    4. Photocopy Your ID:

    Provide a photocopy of government issued photo I.D. for the account owner and authorized signer (if any).  


    5. Fill out Transfer Request (optional):

    If you wish to transfer funds from a Health Savings Account from another financial institution then print, complete and sign the HSA to HSA Transfer Request form found at the link below.


    6. Bring Your Documents and Funds to a Branch:

    Bring these forms with your check for your application fee, opening deposit (if any) and photo identification for you and any authorized signers to any branch location or mail to:

    Please know that all account applications are subject to approval in accordance with its internal policies and procedures. Once the account has been approved, and all applicable fees paid, the account will be opened and a welcome letter including all necessary disclosures will be sent to the address on the account.
